You also have the choice to download the PDF version of both forms by clicking on the names of them below and return them via e-mail to bees@thewholistichive.com, or print them and either mail them to us or bring them to your appointment.

New clients need to fill out both the Informed Consent Form and the PMA Membership Contract.
